Narrative
We transition individuals with disabilities from nursing facilities and other institutions back into the community to live as independently as possible with support services. We do peer counseling, skills training, information and referral, and advocacy. Our services are free to the consumer or the agency that refers us. We can determine what kind of residental options are needed and available, assist with the housing search, make referrals for in-home care, explore and explain tranportation options, assist in finding sources for medical supplies and adaptive equipment, explore and explain financial benefits, assist with finding sources for home/vehicle modifications, provide or arrange for peer support, and continue to be involved after the move takes place.
