A Primary Care practice serves as the first point of entry into the health care system and as the continuing medical home for all health care services. Primary care practices provide:

• Preventive health care (annual visits and physical exams)
• Disease prevention, counseling and patient education
• Diagnosis and treatment of acute and chronic illnesses (sick visits)
• Health-risk assessments, immunizations and screening tests
• Referrals to specialists

Primary Care doctors are advocates for their patients, coordinating the use of the entire health care system for the benefit of their patients.
