Adult Family Care (AFC)
Our Adult Family Care (AFC) services (sometimes called Adult Foster Care) are highly personalized, depending on both the person who needs assistance and the Caregiver, whether or not the Caregiver is a family member.
To become a Member of our Adult Family Care (AFC) program, and be eligible for our program and services, the person who needs assistance must be:
• 16 years of age or older
• Eligible for Medicaid (MassHealth)
* Requires 24-hour live-in assistance
• Needs help with one or more daily activities:
- Bathing
- Dressing
- Walking
- Transferring (Getting in and out of bed)
- Using the Bathroom
- Eating
