Program Name: Lamour Adult Care Program

Blended Strength Based Community and Clinical Programs:

Adult Foster Care & Group Adult Foster Care

Goal of Program

Adult Foster Care (AFC)  services for disabled/elderly adults in their community who has a ordered by a physician to deliver Adult Foster Care in a qualified setting as described in 130 CMR 408.435 by a multidisciplinary team and qualified AFC caregiver, that includes assistance with ADLs, IADLs, other personal care as needed, nursing services and oversight, and AFC care management.

Goal of Program

Group Adult Foster Care (GAFC) services for participants residing in either a site-based, subsidized housing complex (operated by the housing authority) or a certified assisted living facility offering GAFC services

Goal of Program

Our Home-and-community-based services waivers for persons with acquired brain injury seek to serve patients in need of behavioral health services. We reduce and eliminate the barriers for patients in accessing quality mental health care.  Through our Diversionary and Outpatient Program services, we work collaboratively with Home-and-community-based program services to provide linkage to community resources based on needs for a continuum level of care. We provide the following services below

 Goal of Program

Our Adult Companion  services is  directed at companionship, assistance or supervision of the person in the home or community and socialization. We have provided to a participant with non medical services to support  there and independence and address needs . Companions may assist or supervise the participant with such light household tasks as meal preparation, laundry, and shopping. This has helped individual to remain independent in their homes and caregivers a much needed time off. Our models of care support families with participates having significant behavioral health, cognitive, physical, and complex health care needs that are living at home.

Goal of Program

Our Chore services is unusual or infrequent household maintenance task that is needed to maintain the participant’s home in a clean, sanitary, and safe environment. This service includes heavy household chores such as washing floors, windows, and walls; tacking down loose rugs and tiles; and moving heavy items of furniture in order to provide safe access and egress. Due to complex health care needs these services are needed to help participates to live in there home. Example of cases have been significant hoarding can put individuals who hoard and their family members at serious risk of health problems, injury, removal of at-risk children or older adults from the home, homelessness or in the worst case, even death. Our models of care support families with adults having significant behavioral health, cognitive, physical, and complex health care needs that are living at home.

Goal of Program

Individual Support and Community Habilitation services provide community-based to individuals with disabilities a assistance in  developing, maintain, or maximize independent functioning in self-care, physical and emotional growth, socialization, communication, and vocational skill. This achieve with our services by improving health, goal-oriented service which provides more focused and intensive supports in response to identified areas of need and difficulty and which builds and strengthens the individual  participant sand family’s capacity to  support the ability of the participant to establish and maintain a residence and live in the community. Examples of cases include supporting participate learning to make healthy choices in eat and grocery shopping, building natural support and social church activity, learning to cook safely, or learning to use public transportation independently.  We provide services that assist individuals in meeting personal needs and performing basic and complex daily activities. Participates are taught skills that allow them to improve their lives in their local community. Integration of a variety resources and support services are used to have participates learn to access their community in order to increase function and live at home. This service is directed by nursing or professional staff developed care plan or treatment plan. Our models of care support families with adults having significant behavioral health, cognitive, physical, and complex health care needs that are living at home.
