Narrative
Services are tailored to each individual’s need to maximize their potential for independence and provide meaningful community connections. Services include intake and needs assessment, transition planning, individualized service planning, skills development and training, case management, housing, recreation and community access.
Independent Living assists those who have mastered many independent living skills but still need enhanced support. Services include case management and support, assistance accessing community services, training, networking and recreation.
Adult Foster Care provides clinical and financial assistance to qualifying families and caregivers to keep their loved ones at home. Eligible individuals must be at least 16 years of age, receive MassHealth Insurance and require daily assistance with mobility, bathing, dressing, toileting, eating or transferring. Caregivers of individuals who are accepted into this program receive reimbursement determined by the individual’s level of need.
Shared Living provides individuals with the opportunity to become part of a family and a community. Shared Living caregivers are carefully selected and trained to meet the needs and wishes of individuals. Ongoing monitoring and evaluation includes monthly visits by the Shared Living case manager, access to the behavioral clinician, access to the residential nurse and crisis and safety management.
Minute Man also supports individuals living in group residences located in Concord, Maynard, Stow and Littleton. Individuals vary in skill levels and levels of indpendence. All receive support 24 hours a day, 7 days a week.
