Narrative
Advocates Supported Home Care is a certified Adult Foster Care (AFC) program that allows individuals with disabilities who are sixteen or older and elders to live and remain in their own home or in the home of their loved ones, thereby preventing or delaying out-of-home placement. Family members or caregivers receive compensation for providing live-in care to individuals who qualify. Advocates' team of nurses and social workers provides consistent, professional support services to the individual and the caregiver through nursing oversight and individualized, thoughtful care management.
Supported Home Care is a program for individuals who meet the Mass Health eligibility requirements. Our program supports individuals who, because of medical, physical, cognitive or psychiatric problems, cannot safely live alone. Caregivers may be friends, live-in hired caregivers or family members, excluding a spouse or court-appointed guardian.
